Venture Bound Registration and Payment

Registration - July 22, 2024 Deadline
1 - Submit Registration Form

Complete this online Google Form. 

The purpose of this form is to collect information that will help us get ready for your trip. 

2 - Complete Medical History Form

  1. Download the Medical History Form from Google Drive.  

  2. Complete the form.

  3. Submit to your health care provider to review and sign.

Note - This form is specific to Venture Bound. No other UNC Charlotte medical history form can substitute this form.

3 - Return Medical History Form

Return the completed and signed medical history form to Venture Outdoor Leadership.

4 - Submit Payment

Submit payment on our secure, online portal.

Confirming Registration

You should expect to receive a confirmation e-mail if/when you complete registration. 

Refund Policy

Canceled registrations made by July 22, 2024 will receive a full refund. Refunds will not be made for cancellations communicated after this date.  

Please note: This is the same date as the registration deadline.
